Pituitary cystic images: difficult diagnosis

Introduction: The most frequent cystic lesions in the pituitary region are: cystic adenomas, Rathke’s pouch cysts and craniopharyngiomas. But sometimes the diagnosis can be difficult.

Clinical case: This is the case of a women patient who consulted at the age of 20 for a pituitary cystic process revealed by secondary amenorrhoea; evoking in the first place a Rathke’s pouch cyst. Hormonal assessment reveals tumour-related hyperprolactinemia; the diagnosis of cystic prolactinoma is evoked. It is the response to anti-dopaminergic medical treatment and the morphological response to MRI that leads to the diagnosis of a probable cystic prolactin adenoma.

Conclusion: A radiological image can only be interpreted with caution because it is the set of clinical and biological characteristics that allow a definitive diagnosis to be made.
