Introduction
Masked arterial hypertension (HTAM) is a relatively recently described entity which gives patients with it the same cardiovascular risk as permanent hypertension. In this context, we report a prospective study including 53 patients in order to determine the prevalence and predictive factors of HTAM in diabetics.
Methods
Descriptive prospective study of 53 patients treated in the endocrinology Diabetology departement of hedi ckeker university hospital sfax, Tunisia. These patients had no history of hypertension and did not take any treatment for high blood pressure. Medical follow up showed normal blood pressure (lower than 140/90) for the last 2 years.
Results
The average age of our patients was 55.3 years (range 3575 years) with a sex ratio (M/F) equal to 0.89. The duration of diabetes was on average 8.7 years (range 217 years). The prevalence of HTAM was 64%, occurring in predominantly non-dipper subjects (58.5%) Regarding the predective factors of HTAM, we were able to collect in univariate analysis the following factors: duration of diabetes, fasting blood sugar, weight and microalbuminuria. In multivariate analysis, the predictive factors that emerged in our study are poor glycemic control (HbA1c > 7%), high body mass index (BMI) and duration of diabetes with an OR of (15.8; P = 0.04) (1.2; P = 0.02) and (1.3; P = 0.009). Our results are consistent with those of the literature which shows that diabetes imbalance, BMI and duration of diabetes are predictive factors of AMTH. Other risk situations for HTAM, not isolated in our work, have been found and mentioned by some authors: advanced age, male sex, metabolic syndrome as well as normal high blood pressure in the office.
Conclusion
It seems essential to carry out ambulatory screening for HTAM in diabetic patients. The induced morbidity and mortality and the cardiovascular risk would be significant and fully justify it.
