A comprehensive pregestational prenatal monitoring and early detection of diabetes amongst pregnant patients is the cornerstone of the updated ADA recommendation 2016 in Diabetes Care.
It is our main objective to lay out an attainable management plan for early diagnosis, and management of gestational diabetic in a community clinic.
The plan includes implementation of family planning and effective contraception for known women with previous gestational diabetes, as primary preventive measure.
The design consists of reeducation and reinforcement of gestational diabetes awareness and also of monitoring the following targeted A1C as per recommendation of a target of 66.5% (4248 mmol/mol), although depending on hypoglycemia risk the target may be relaxed or restricted. Glyburide in gestational diabetes mellitus was deemphasized based on new data suggesting that it may be inferior to insulin and metformin in accordance with the ADA recommendation.
In conclusion consistent and regular reeducation, blood A1c, FPG, RBG monitoring, diet, weight monitoring, and adherence to medication compliance, and highlighted emphasis on the updated ADA recommendation was noted to be effective and shows achievable targets to community patients afflicted with gestational diabetes.
