Introduce: Lack of radioiodine uptake in Differentiated Thyroid Carcinoma (DTC) is a huge diagnostic and therapeutic problem. This sign is associated with worse prognosis. In patients with elevated thyroglobulin levels and with no evidence of disease on radioiodine scintigraphy, scintigraphy with somatostatin analogs can be an alternative imaging modality.
The aim: Assessment of scintigraphy with the somatostatin analog 99mTc-HYNIC-TATE to visualization of non-radioiodine-avid DTC. All patients gave written informed consent to participate in the study. The local Ethical Committee approval has been obtained before.
Material and method: Four patients with metastatic non-radioiodine-avid DTC (3 with papillary thyroid carcinoma (PTC) and 1 with follicular thyroid carcinoma (FTC) underwent neck, chest and upper abdomen scintigraphy with the somatostatin analog 99mTc-HYNIC-TATE produced by OBRI POLATOM Świerk/Poland. The scintigarphy image acquisition was performed with a double head gamma camera Varicam Elscint in 10 min, 2-3 h and 24 h after i.v. injection of 99mTc-HYNIC-TATE preparation in dose 15-20 mCi. The SPECT images were reconstructed with use of HERMES (Nuclear Diagnostic, Sweden) processing workstation.
Results: A pathological uptake of 99mTc-HYNIC-TATE was found in every patient with PTC
In patient with FTC there wasnt observed any accumulation of this radiopreparation.
Conclusion: Non-radioiodine-avid Differentiated Thyroid Carcinoma can be visualized with 99mTc-HYNIC-TATE preparation.
